As part of the grant application you will be required to select a minimum of 3 outcome measures. To assist you in identifying the appropriate outcomes for your program, please refer to the Outcome Measurement Reference Guide.
Please note, beginning October 1, 2017, PUCF will be changing its grant cycle periods.

If you are a previous recipient and have not submitted a report for your 2016 grant, you will be required to submit a summary report before reapplying for a new grant. Please use this PUCF Progress Report Template and attach it to your new grant application.
Due to a new grants management system, each organization applicant will be required to create a new account. You will then receive a validation email. Once you've validated your account you will need to come back to this web page and click the "Apply Now" button again to return to the grant application portal.
